Medical Records & Release Forms

We keep a private, secure medical record about your health.  Your medical records are maintained for up to 10 years post last date of care or graduation.

We take evey precaution to keep these records secure and in order.  Our Notice of Privacy Practices explains the ways we may use or disclose (release) your medical records.  Contact us if you have any questions.

Note: To protect the confidentiality of our students, we do not e-mail copies of medical records with the exception of immunization records.

You can:

  • Request a copy or parts of our medical record for yourself or others.  There is a $15 fee for this service.
  • Request a copy of just your immunization record for yourself or someone else.  No fee associated with this service.  
  • Review the information in your medical record.

Release of Information

Please fill out a Release of Information if you would like to:

  1. Authorize Dartmouth College Health Service to Release Records 
  2. Authorize Dartmouth College Health Service to Request Records from another provider
  3. Authorize Dartmouth College Health Service to discuss medical treatment with a third party

Immunization Records Only

No Fee

Requests generated from non-Dartmouth issued email addresses may require photo ID

  • If you are a current Dartmouth student you can access and print your immunization record through Dart Hub under the 'Health Center Online' link, or you can stop by the Reception Window at Dick's House to request a signed copy.

Processing Fees for copies of your medical record

There is a $15 processing fee for this service.

  • If you are not on campus payment can be made in the form of a check made payable to 'Dartmouth College Health Services', or credit card over the phone.
  • If you are on campus we accept cash, check or credit card payments at the Reception Window at Dick's House. Current students also have the option of charging the processing fee to their Student Account.
  • Requests for medical records will not be processed until both the authorization and payment have been received.

Processing time is 7 - 14 business days. If your request is urgent plese call the Medical Records Office at 603-646-9405.
