Dependent Enrollment

Dependent enrollment in DSGHP is optional and must be renewed on an annual basis during the open enrollment period. To enroll a dependent in DSGHP, the student must be enrolled in DSGHP. For enrollment dates/deadlines, and to see rates for dependent coverage, please refer to our Premium Costs & Coverage Dates page.

• Dependent Eligibility:
  • Eligible dependents include the following:
    • Spouse or Domestic Partner
    • Child (biological, adopted, foster or court appointed, or stepchild of student or enrolled spouse/domestic partner) up to age 26
• How to Enroll Dependents:
  • Complete the Dependent Application ​​​​​​​form and return it to the DSGHP office via email (see DSGHP Forms).
