Returning Upperclassmen

A Note Regarding Academic Year 2024-2025

The online Medical History Questionnaire for upperclass athletes returning in the 2024-2025 academic year will be accessible around July 1, 2024. We will contact you through ARMS when these forms are ready to complete.

 

A repeat physical exam is NOT required for returning upperclassmen.

 

Medical Clearance Requirements

Returning student-athletes are required to have a physical exam prior to their first year of participation in varsity sports or Men's Rugby at Dartmouth. A repeat physical exam is NOT required for returning upperclassmen. If you are uncertain whether you have an athletic physical on file, please contact the Sports Medicine office.

For returning upperclassemen, the following items are required for the upcoming academic year, 2024-2025:

  • Complete our Online Medical History Questionnaire [available summer 2024].
    1. Log into the Health Portal
    2. On the left side of the screen select: "Forms"
    3. From the list of Forms select: "SM Athletes Only - Returning Athlete History Form"
      • Provide as much detail as possible regarding previous injuries or illnesses.
      • Once completed, click "Submit Final" at the bottom of the questionnaire.
      • You will get a message that the questionnaire has been successfully completed; if you receive an arror message at this step, please contact our office before attempting to resubmit.
  • Complete annual waivers, releases, and online forms, also accessed in the Forms tab of the Health Portal.
    1. Athletics Assumption of Risk and Waiver of Liability
    2. Health History Agreement and Attestation
    3. Consent and Authorization for Use
  • Provide documentation for use of any banned prescription medications, including for ADD/ADHD (if applicable).
    • For details on what is required, review the Banned Substance Documentation Form.
    • Documentation of the use of these banned medications is required to be re-submitted annually as long as you continue to participate in NCAA athletics.
    • You must provide specific documentation of diagnosis and treatment to allow for medical exception.
    • Refer to their website to read more about the NCAA drug testing program.
    • A positive drug test could keep you from participating in your sport for up to one year.
