Case Management services include

  • Supporting and regular check-ins to help prioritize academic and medical needs  

  • Assisting students in navigating campus and community resources, making referrals when appropriate  

  • Sharing institutional knowledge to help students make educated academic decisions 

  • Understanding campus trends, designing support networks, and coordinating resource delivery.  

  • Collaborating with individuals who are a part of a student's support team  

  • Helping students develop plans to overcome barriers to academic success  

  • Following up with stakeholders to ensure support is received promptly and concerns have been effectively addressed. 

Our Process

  • Case managers collaborate with students assigned to undergraduate deans to ensure effective and timely support. In most situations, students are referred to Case Management by their assigned undergraduate dean or other faculty and staff members at Dartmouth College. Case Management evaluates the suitability of each referral and intervenes when appropriate. If deemed necessary by Case Management, an initial meeting is arranged with the student to establish an action plan to address their needs. Support may involve coordinating access to campus and community resources, constructing plans to foster resiliency, and promoting self-advocacy. Subsequently, Case Management follows up with students to confirm the successful resolution of the issues at the time of referral.    
  • Case Management chairs weekly Campus Partners and Behavioral Assessment Team meetings. Campus Partners meetings aim to detect student concerns before they become crises, and the Behavioral Assessment Team assesses and intervenes in critical student situations.   
  • Case managers connect with students following hospital admission. These interactions may include contacting students while they receive care, partnering with students to communicate with faculty, and creating a plan to progress academically. Case Management prioritizes each student's unique needs and experiences, recognizing their strengths and challenges. Thus, support is individualized to each student's needs.
